On your first visit upon entering our office our receptionist will greet you and welcome you as a member of our family. We request that you complete the New Patient Paperwork ahead of time. These forms can be completed at home. Our detailed paperwork provides us with important information about yourself and your condition. Plan on being with the doctor for two hours on your first visit.
